- Claims paid fast — for out-of-pocket expenses, Cigna's reimbursement is typically within 10–14 business days. Several Phuket expats I know report faster turnaround than that.
- Non-OA visa compliant — the Silver plan with Thai cover easily meets the ฿40,000 OP / ฿400,000 IP minimum requirements.
- Good customer service — Cigna has a 24/7 English helpline that is actually useful, not just a phone tree. This matters at 2am in Bangkok Hospital.

What to watch out for
- Price — Cigna is the most expensive of the three for equivalent coverage. At age 50+ the premiums become significant.

What's good
- Lower premiums — typically 15–25% cheaper than Cigna for comparable coverage. This can mean a saving of ฿8,000–฿18,000 per year for a 40-year-old.
- Direct billing at Bangkok Hospital and Siriroj — AXA does have direct billing in Phuket, though the setup process is reportedly slightly more admin-heavy than Cigna on first use.
- Worldwide coverage — AXA IPMI plans cover you globally, which is useful if you travel frequently outside Thailand.
- Strong inpatient coverage — hospitalisations and surgical procedures are generally handled well. Where AXA gets more complaints is outpatient claims.
What to watch out for
- Slower claims reimbursement — AXA's outpatient claims can take 3–6 weeks, longer than Cigna. For routine GP visits this is inconvenient but manageable; for expensive dental or specialist visits it means more out-of-pocket float time.
- Product complexity — AXA has rebranded and restructured its Thailand IPMI products multiple times. Make sure you're buying the right product through a licensed broker, not a legacy plan.
- Premium increases at 60+ — AXA's premiums step up noticeably after age 60 and again at 65. If you're in your late 50s, model the 10-year premium trajectory before committing.

Good Budget Option — Read the Small PrintHere's the critical thing: "BUPA Thailand" is not UK BUPA or Australian BUPA. It's operated by a Thai company under licence from BUPA UK. The products, claims processes and customer service are different. Many UK expats buy BUPA Thailand assuming it works like home — it doesn't always.
What's good
- Lowest premiums of the three — for basic inpatient and outpatient coverage, BUPA Thailand is typically the most affordable major-brand option.
- Reasonable hospital network — covers Bangkok Hospital Phuket, though the direct billing experience can be less smooth than Cigna.
- OIA approved — BUPA Thailand plans meet the Non-OA visa health insurance requirements.
What to watch out for
- Annual limits can be low — entry-level BUPA Thailand plans may have ฿3M annual limits. Given that a serious accident at Bangkok Hospital Phuket plus medical evacuation could cost ฿2–5M+, this is uncomfortably tight.
- Pre-existing condition handling — several expats in Phuket Facebook groups have reported disputes at renewal around pre-existing conditions. Read the policy wording carefully and keep full documentation of all your health disclosures.
- Not the same as UK/Australian BUPA — I keep repeating this because it catches people every year. If you're an existing UK or Australian BUPA member, your home policy almost certainly does NOT cover you in Thailand long-term.

The Non-OA Visa Insurance Requirement
If you're applying for or renewing a Thai Non-OA retirement visa, you need health insurance that meets specific minimum thresholds. The requirement, unchanged since 2019, is:
- Outpatient coverage: minimum ฿40,000 per year
- Inpatient coverage: minimum ฿400,000 per year
- Insurer must be on the OIA (Office of Insurance Regulation) approved list
Cigna, AXA and BUPA Thailand all offer plans that meet these minimums, but you must confirm OIA approval status at the time of purchase as the approved list is updated periodically. The minimum coverage is also honestly quite low — ฿400,000 inpatient won't cover a complex hospitalisation at Bangkok Hospital Phuket. I'd recommend ฿2–5M minimum inpatient, not the visa-mandated floor.
